BVDC Loyalty Plan

No yearly maximums, no deductibles, waiting periods, and no pre-authorizations!

How do I sign up? Please stop by our office to enroll today.  Your membership activates as soon as your membership fee payment is received.

How long is my membership? Your membership is 12 months starting the day your plan is activated.

Can I sign up my whole family? Yes, we love seeing families at Boulder Valley Dental!

I am already a current patient at BVDC.  Can I join? Yes! New and existing patients are both eligible for the BVDC Loyalty Plan.

I have a FSA/HSA plan, can I submit the plan membership fee for reimbursement? Yes!  You are welcome to submit your plan fee for FSA/HSA reimbursement.  We recommend you contact your employee benefits provider confirm the details of their coverage.

I have dental insurance. Can I sign up for this plan? The BVDC Loyalty Plan is not valid in combination with your existing dental insurance plan.  We sincerely appreciate your loyalty to our practice!

How can I use my discount for dental treatment? To receive care, simply call our office for an appointment on or after your effective membership date.

How do I cancel my membership? The BVDC loyalty plan is an awesome benefit, rarely does anyone cancel.  However, we understand certain circumstances arise and do allow cancellation within the first 30 days of activation.  If you cancel within the first 30 days, you will receive a full refund of the membership fees.  Also, you will be required to pay full fee for the services rendered within the first 30 days of your membership prior to cancellation.  The annual fee will auto-renew after one year.  Please contact Boulder Valley Dental to make changes to your plan membership.

Are there any Treatment exclusions? There are no treatment exclusion for elective care with our loyalty plan!  Teeth whitening? Porcelain Veneers? Dental implants? Clear braces? CBCT 3D scans? Dental products such as Waterpiks? They are all covered under the plan.  Did we mention…No Exclusions!

What happens if I miss an appointment? Loyalty plan members are responsible to maximize benefits by scheduling appropriate appointments within the 12 month membership period. We recommend reserving your dental hygiene appointments 4-6 months in advance.

Please notify our office at least 48 business hours in advance if you must reschedule a reserved appointment time.  Missing reserved appointments or cancelling with less than 48 hour business day notice of reserved appointment time results in forfeiting your dental exam/cleaning benefits for that appointment. Benefits do not roll over and must be used within the 12 month period.
